What an Ontario Health Team means for South Simcoe
Stevenson Memorial Hospital, in collaboration with 13 local health care providers has submitted a self-assessment to become an Ontario Health Team (OHT) for South Simcoe. Partners in the potential South Simcoe OHT include:
- Matthews House Hospice
- Alliston Family Health Team
- Simcoe Manor
- Nurse Next Door Home Care Services
- Addiction Services for York Region
- York Support Services Network
- LOFT Community Services
- Community and Home Assistance to Seniors (CHATS)
- Krasman Centre
- Canadian Mental Health Association
- Closing the Gap Healthcare
- Comfort Keepers
- Dr. Oswaldo Ramirez (Independent Physician)
The status of the submission will be confirmed in July.
