SMH Launches Stevenson@Home Program
[New Tecumseth, ON – November 30, 2023] – Stevenson Memorial Hospital (SMH) is announcing a new program in partnership with Bayshore HealthCare called Stevenson@Home. The program will allow eligible patients to receive care at home once discharged from the hospital.
The program will benefit both patients, families and the hospital as patients can continue to receive care they need in the comfort of their homes to enhance their recovery, while supporting hospital capacity and patient flow needs.
Alternative Level of Care (ALC) patients continue to occupy beds in hospitals across the province, and SMH is no different. ALC patients are those no longer requiring acute care, but are unable to be discharged as they don’t have the supports they need at home to live safely and recover fully, or they are waiting for a long-term care bed.
“Our capacity continues to be challenged with increased patient volumes and high percentage of ALC patients,” says Julia Mullen, VP Clinical Services & CNE, SMH. “With the Stevenson@Home program, we are better able to support patients with their recovery process, allowing them to go home sooner with the supports they need. We are very thankful for this program and partnership with Bayshore HealthCare which will benefit our hospital and community,” she adds.
The Stevenson@Home team consists of a care transition navigator, nurses, personal support workers, occupational therapists, physiotherapists, social workers and dietitians in partnership with Bayshore HealthCare. Patients may stay with the Stevenson@Home program for up to 16-weeks.
Eligible patients and families will meet with SMH staff to discuss enrollment in the program, what to expect, and work with them in consultation with Bayshore HealthCare staff to create a care plan identifying the specific services the individual may need at home, allowing for a seamless hospital discharge and home care experience.
