Partnering with Patients and Families to Improve Care

Stevenson Memorial Hospital is committed to providing high-quality, safe care to patients and families when they need it most. We continuously strive to improve the care and services we provide by partnering with those who use them. The insights and perspectives of patients and families help us identify opportunities for improvement and ensure that care remains responsive to the needs of our community. Patient and Family Advisors provide valuable input on programs, policies, services, and projects that impact patients and families. Advisors are encouraged to share their perspectives and contribute to discussions on topics such as:

  • Access to care and services
  • Patient and family information and communication
  • Program and policy development
  • Facility design and redevelopment projects
  • Signage and wayfinding
  • Quality improvement and safety initiatives
  • Planning and delivery of care and services

About Our Patient and Family Advisory Council 

The Patient & Family Advisory Council (PFAC) brings together patients, family members, hospital staff, and physicians to improve care and services at Stevenson Memorial Hospital. Patient and Family Advisors share their perspectives and provide feedback on programs, policies, projects, and initiatives that impact patients and families. Some advisors may also choose to serve on committees or participate in working groups focused on specific improvement initiatives. By partnering with the healthcare team, advisors help ensure that patient and family voices are included in decisions that shape care at Stevenson.

Would you like to help shape the future of healthcare in your community?

Community members can make a meaningful difference by becoming a Patient and Family Advisor. Advisors work alongside hospital leaders, staff, and physicians to provide feedback, share perspectives, and contribute to initiatives that improve care for all patients and families.

Eligibility

To become a Patient and Family Advisor, applicants must: 

  • Be a patient or family member who has received care or services at Stevenson Memorial Hospital within the past three (3) years
  • Be interested in partnering with hospital leaders, staff, and physicians to improve healthcare services
  • Be able to share perspectives respectfully and constructively, and maintain confidentiality.
  • Commit approximately two (2) hours per month

Building a diverse PFAC helps ensure that the needs and priorities of our communities are reflected in the planning, delivery, and improvement of healthcare services.

  2. Submit your completed application by email to patientexperience@smhosp.on.ca
Applications are reviewed by a selection committee. The selection process may include a brief interview and reference checks. A satisfactory Criminal Record and Vulnerable Sector Check may also be required.

All applicants will be notified of the outcome of their application.
